Even if you have other coverage, you will benefit from this plan because you can combine them together to get even more coverage. In addition, you can cover your eligible family members as well. For details contact: Olga Sauer @ (519) 661-3394. ANNUAL BILLING *NEW* SOGS has moved to an annual billing cycle for the health and dental plan. This is on par with the procedures at many Canadian Universities. Students will be assessed their annual premium in September 2009. All students who do not opt-out in September will have access to their plan benefits through August 31, 2010. That is, students who leave Western before August 31, 2010 will be able to continue to use the plan; it will not terminate upon your completion date (or withdrawal date) at Western. IMPORTANT Claim Submission Information UWO Society of Graduate Students Group Health and Dental Plan is insured by RWAM Insurance Administrators. An active student on the Health and Dental Plan has 1 year to submit a claim after a service has been rendered. A student who has graduated, and their coverage has terminated, they have 3 months to submit any eligible claims for reimbursement. Prescription Drug Claims Green Shield Canada is RWAM's drug card carrier. Prescription claims can be billed on-line by your Pharmacist directly to Green Shield using your NEW RWAM card with the "SGS" pre-fix followed by your Student # - 00. If you pay for your drug claim, submit a completed health claim form and the original drug receipt for reimbursement to the SOGS Office (UCC 260) or directly to RWAM at the address below. Dental Claims Your Dentist can bill RWAM directly using the information on the BACK of your pay direct drug card. If you do pay for your dental claim, submit a completed Standard Dental Claim Form for reimbursement to the SOGS Office (UCC 260) or directly to RWAM at the address below. All Other Claims Please pay for all other types of claims (i.e., vision, massage, practitioners, etc.) and submit a completed health claim form and the original drug receipt for reimbursement to the SOGS Office (UCC 260) or directly to RWAM at the address below.
